Note:  The Handbook for Students and Parents currently published contains information for  2009. For example, the lodging arrangements at the WVU session are different. The Honors Hall, although very close to the science building, is further from the Mountainlair.As a result, students will not be allowed to walk to the Mountainlair unless they are accompanied by an adult.

A Handbook for Students and Their Parents (2009) includes information about the summer experience upon which you are about to embark. It should answer questions about West Virginia University, residence facilities, what to pack, what to leave at home, rules and regulations.

Please read the Handbook before completing the forms.

Directions for downloading, completing and mailing forms

The forms are published as portable document files (pdf).  Download, save and print them. Complete the form clearly, using a black pen. Incorrectly completed or illegible forms will be returned to the student. There are two forms--a total of three pages.
